Families in Transition merged with New Horizons as of January 1, 2018. Families in Transition is one of the largest homeless services organizations in New Hampshire with locations in Manchester, Concord, Dover and Wolfeboro. We provide innovative and effective interventions specifically designed to help homeless individuals and families reach beyond the cycle of homelessness to lead healthy and successful lives. Our belief is that having a home is a basic human right and is fundamental to becoming an engaged and contributing member of our community.

In 2020, we provided:

    • shelter to 1,544 individuals, of whom 399 were children.
    • longer-term, supportive service housing to 456 people, of whom 229 were children.
    • substance use treatment to 177 individuals at our Willows Substance Use Treatment Center.
    • 196,915 meals.
